The mission of the Sandwich Community Nurse Association (SCNA) is to fill gaps in care not provided by Visiting Nurse Associations, primary care or home caregivers.
The goal is to help our residents stay healthy and at home.
Sandwich Community Nurse services are free to Sandwich residents, year-round and seasonal. Please remember the community nurse is not an urgent care or emergency service.
For urgent needs, contact your primary care provider and in emergencies call 911.
Welcome our Community Nurse, Danielle Paquette-Horne
Danielle Paquette-Horne, RN, will be available on Wednesdays
and Thursdays, at no charge to Sandwich residents, to:
♦ Make home visits
♦ Evaluate health care needs
♦ Help find health care resources
♦ Offer health care education
♦ Reduce social isolation
♦ Help communicate between individuals, their families and caregivers
Danielle has extensive experience in home care, inpatient and long-term care, and community nursing. She will also develop educational programs for Sandwich residents.
Who might want to call Danielle? Sandwich residents who:
♦ Have a new baby in the family
♦ Just got out of the hospital or outpatient surgery
♦ Are unsure how to take medications
♦ Are worried about managing blood pressure, diabetes, other chronic conditions
♦ Have had a close call with a fall at home
♦ Need help getting caregivers/family on the same page
